Overview: The Field Intake Form was created to streamline requests regarding Route single storage units. In response to Zendesk tickets – this form will be sent. It is a user-friendly form that will capture all information so that we can action issues quickly. This form should be used by Regional Managers for all storage-related open/close activity going forward.

Request: New Unit (Primary)
- Purpose: New unit, new vendor.
- Next Steps: DSD Home Office partners will review the request then contact the RM to discuss preferred locations and review your options

Request: New Unit (Secondary - established location/vendor)
- Purpose: Secondary unit at established vendor location
- Next Steps: DSD Home Office partners will reach out to the vendor to secure an additional unit. RMs will be CC'ed in the communications to provide additional information, as needed

Documentation for Moveout
- Purpose: confirmation of vacated unit and documentation for vendor
- Next Steps: DSD Home Office partners will submit
NOTE: This is not applicable to full warehouse closures needing multiple units - team to handle directly with RMs

To access the form using a direct link, please click Here: Field Intake form

Click the Form Submission Types drop down to view the available options to select:
- REQUEST: New Unit (Primary)
- REQUEST: New Unit (Secondary/Additional)

PLEASE NOTE: Based on the option selected, there will be different fields to fill out. Please fill out each required field. When entering the Route number, please enter the three digit route number, for example:
RTE #1 = 001
RTE #42 = 042
RTE #302 = 302
